I wish to undertake the services of

and agree to the following terms and conditions therein:
Firstly, if you are under 18, the Studio cannot proceed with the service without the consent of a parent or guardian.
1. I acknowledge and affirm that I am in good physical condition and have no medical condition that would limit or prevent my use of the services, facilities and products of Total Cover Plus, nor do I have any skin condition that may react to the treatments/facilities provided by the Studio and Total Cover Plus.
I have consulted with a physician who approves my use of Total Cover Plus hair systems and its related services and facilities, including without limitation shaving and washing of the scalp, conditioning of the scalp, bonding of a hair system, washing and conditioning of hair and hair colouring, facial treatments, hair cutting and styling.
We will need to ask you some health-related questions to ensure a safe service and to provide industry-specific advice, as this treatment may not be suitable for you if you suffer from certain health conditions or are on certain medications, despite the approval of a physician.
Where we collect information about your health or medical circumstances, we have a legal obligation to collect this information for health & safety and insurance purposes and only do so with your explicit consent.
The privacy and security of your data is very important to us, and we will only use health-related information for the purposes described in this form and in our Privacy Notice.
2. I further acknowledge I am aware that there are potentially dangerous conditions present at the Studio, including, without limitation, equipment including scissors and razors, hot and cold temperatures associated with the Studio’s services, chemicals, slippery equipment and floor surfaces as a result of the presence of water, soap, and/or oils.
3. I am aware of the risks involved in and its related facilities and services. With that awareness in mind, I voluntarily agree to use the services at the Studio and Total Cover Plus and its related services. I knowingly assume all such risks and accept personal responsibility for any damages following any injury to my person, including without limitation any injury that results in permanent disability or death.
4. As further consideration for the privilege to use the Studio and its related services, I agree, on behalf of myself, my spouse, domestic partner, heirs, assignees, guardians, or legal representatives, that no claim will be made against the Studio and Total Cover Plus or any of its officers, directors, employees, agents, independent contractors, shareholders, insurance companies, attorneys, affiliates, renters, subcontractors, vendors, landlords, lessors, and/or its successors in interest for any injury, property loss, and/or damages resulting from my presence at and use of the Studio and Total Cover Plus and its related services and facilities, including use of any parking lot operated by the Studio or its vendors.
5. I agree to indemnify, defend and hold the Studio and Total Cover Plus, or any of its officers, directors, employees, agents, independent contractors, shareholders, insurance companies, renters, attorneys, affiliates, subcontractors, vendors, landlords, lessors, and/or any successor in interest to any of the foregoing, harmless from all claims, actions, or demands, whether or not such matters are filed in a court with proper jurisdiction or through arbitration, that may result from my use of the Studio and Total Cover Plus and its services and facilities, including, without limitation, claims for bodily or emotional injury, sickness, loss of property, and other damages.
6. I acknowledge that the Studio has the right to refuse to allow the use of its service and facilities to any person whose conduct is deemed by the Studio, in its sole and absolute discretion, to constitute harassment or that is offensive, inappropriate or is causing or deemed by the Studio to be likely to cause an unreasonable disturbance to the Studio’s other clients, guests, vendors, employees, agents and/or independent contractors and that any such conduct so deemed by the Studio will result in my expulsion and termination of my use of the Studio and its services and facilities, without refund of money advanced for the use of the Studio’s services and facilities.
7. I acknowledge that the Studio has the sole and absolute discretion to refuse to allow the use of its service and facilities to any person whose conduct is deemed by the Studio, to constitute harassment, offensive, inappropriate or is causing or deemed by the Studio to be likely to cause an unreasonable disturbance to the Studio’s other clients, guests, vendors, employees, agents and/or independent contractors and that any such conduct so deemed by the Studio will result in my expulsion and termination of my use of the Studio and its services and facilities, without refund of money advanced for the use of the Studio’s services and facilities.
7. I understand that the Studio will always try to ensure my appointment is with my requested stylist; however, I understand this is subject to change on the day due to availability; no stylist can be guaranteed, and the Studio can not be held responsible for this.
CCTV Policy
Within its facilities, The Studio believe that CCTV plays a legitimate role in helping to maintain a safe and secure environment for all members of staff, customers, potential customers, employees of its partners and suppliers and contractors. Images recorded by CCTV are Personal Data and as such will be processed in accordance with data protection laws. All our studios are committed to complying with its legal obligations in order to appropriately handle and protect Personal Data and ensure that the legal rights of staff, guests, customers, and potential customers, employees of its partners and suppliers and contractors relating to their Personal Data, are recognized and respected.
Arrival Time – Cancellation – No Show – Late Arrival
Please arrive 5 minutes before your appointment. It is important that the Studio can offer its clients the service they deserve.
Client appointments are essential to the Studio. The time allotted for an appointment is reserved specifically for the client to enjoy their services and get the full attention of their stylist. The Studio understands that sometimes unavoidable situations arise, and schedule adjustments are necessary. Of course, everything possible will be done to work with the client in the event of an emergency. However, the Studio respectfully requests at least a 24-hour notice to adjust or reschedule appointments.
When booking an appointment, we require a 50% deposit of your total service appointment cost. This will be taken off of your bill when you attend your appointment. If you do not cancel your appointment within 24 hours, this deposit is non-refundable and will not be transferable to another appointment.
If 209 Hair Studios cancels an appointment with less than (24 hours notice), a new appointment will be scheduled without penalty to the client, subject to availability.
The cancellation policy applies equally to no shows. The Studio reserves the right to refuse service and/or the use of its facilities and services to clients who regularly no show, in special situations, we may ask for 100% deposit that will not be refundable should you cancel.
Out of respect for all clients, guests arriving after the start time of their appointment may be asked to reschedule. When possible, the Studio will do its best to alter the services to offer its clients an appointment upon your arrival.
Items left at the Studio
Please note that the Studio can not be held accountable for any personal belongings left at the Studio. The Studio will do all it can to keep any belongings left in a safe place, and the Studio will do all it can to reunite any items left behind. Still, the Studio or its directors can not be held responsible. The Studio, directors or any subsidiaries will offer no monetary or replacement in the case of items that have been disposed of.
This includes any previously worn systems; the responsibility to keep any removed systems that have not been subject to the money-back guarantee is up to you as the system wears and, if left, will be disposed of at the end of your appointment.
Purchases
Returning goods
If you want to return an item, please contact the Studio first. Once the return has been approved, please abide by the following:
· The product should be unused and unopened (unless damaged).
· The product should be returned in original wrapping/packaging.
Any products returned which contravenes this policy will not be liable for a refund or replacement.
Products Excluded from Returns
The Studio is unable to accept returns on any liquid-based product which include, but not limited to, Shampoo, Conditioner, Bond, Hair Styling Products. If the Studio deems the packet as open, the Studio reserve the right to withhold the refund.
Services
Client satisfaction is essential to for our studios.
If clients have questions or concerns about a service, they are receiving or have received they should not hesitate to reach out to the Studio.
Monetary refunds will not be made for hair system services rendered; however, the Studio will happily make adjustments to the original service. Requests for hair system (application or refit), haircut or hair colour adjustments must be made within seven days of the initial appointment. Monetary refunds relating to hair systems will be subject to Total Cover Plus Money-Back Guarantee own policy.
Model services are not eligible for future adjustments or monetary refunds of any kind
Bespoke Hair System
Bespoke hair systems are items that are individually made to the specifications of a client. On average, the production time of all Total Cover Plus hair systems is 12-14 weeks. Based on the specifications (such as size, hair length, texture, wave pattern and density) these production times may vary. Other public holidays, such as Christmas, New Year, Chinese New Year etc. may lead to an extension of the production time.
Thus, production times cannot be guaranteed.
To avoid disruption and the availability of bespoke hair systems, Total Cover Plus and the Studio advice clients to order at least two bespoke hair systems at a time.
Bespoke hair systems will only be put in production once the payment in full has been taken at the Studio or a Flatrate confirmed by Total Cover Plus has been put in place.
The Studio will keep clients updated on production times and progress.
Non Total Cover Plus Systems
As a studio, we work exclusively with Total Cover Plus hair systems and their products.
From time to time due to the nature of what we do we may have to remove a non Total Cover Plus system as part of a consultation or as an interim maintenance appointment while waiting for the delivery of a bespoke Total Cover Plus system.
The Studio and Total Cover Plus hold no responsibility when working with non Total Cover Plus systems, as the client you assume all such risks and accept personal responsibility for any damages to any non Total Cover Plus system, and you are aware of the risks involved in and its related facilities and services.
Products
The Studio will only recommend or provide (advice on) products that have been tested with Total Cover Plus hair systems.
Deposits
Deposits will be taken for all appointments.
Full payment will be required on all bespoke systems.
Refunds of deposits for standard hair systems are subject to the cancellation policy.
Payments for bespoke hair systems are not refundable.
Money-Back Guarantee
First-time customers are eligible for the Money-Back Guarantee. The Studio and Total Cover Plus will refund the full cost of the hair system or Flatrate payment (not the service charge) if the client returns their hair system to the Studio within five working days after the initial application. At the Studio, the client will be asked to fill out a return form so the Studio and Total Cover Plus can learn the reasons behind their decision. After that, a refund will be processed within seven days.
Total Cover Plus hair system Warranty
The longevity of a hair system relies on the individual circumstances of the individual client. This means the Studio and Total Cover Plus cannot give any warranties or guarantees in regard to the longevity of the hair system. The better the client looks after the hair system, the longer it will last and look good.
The actual lifespan of the hair systems may vary considerably depending on how the client treats and maintains the hair system.
The longevity of the bond relies on the individual circumstances of the individual client. This means the Studio and Total Cover Plus cannot give any warranties or guarantees in regard to the longevity of the bond.
The individual body chemistry, lifestyle and environmental factors can influence the longevity of the bond considerably. When the bond stops working, the hair system will require rebonding. Rebonding is considered part of the adequate maintenance of the hair system and subsequently has an impact on the longevity of the hair system itself.
Based on long term experience with thousands of hair system wearers the Studio and Total Cover Plus provide an average longevity expectancy level to guide new clients towards a realistic expectation.
On average most clients rebond the hair system once a month and their hairline regularly every 2 weeks.
The actual lifespan of the bond may vary considerably.
The use of Total Cover Plus Bond
A safety data sheet for the bond can be obtained at the Studio.
It states the results of the toxicological analysis, which attests the bond’s classification as non-hazardous to health.
It is non-biodegradable but not harmful to the environment. The EC Classification is Non-Hazardous.
The PH level is depending on temperature 6-8.

Clients with Disabilities
The Studio provides equal access to all of its services for disabled clients. The Studio will provide personal assistance to ensure that disabled clients have full and complete use and enjoyment of the Studio’s services.
Liability for Damages
Clients shall be responsible for all damages to the facilities caused by client and client’s family members and/or guests. Client agrees that the Studio may charge the client’s credit card for any such damages.
I have carefully read the above user agreement, waiver and release of liability and understand that by signing it, I am waiving substantial rights, and with that knowledge, I sign it voluntarily.
